► Pick up a “Medical Authorization/Physical Packet” from the Athletics Office or Athletic Training Room in the Field House.

► Have you and your parent/guardian complete and sign the Medical Authorization Packet/Medical History portion of the Physical Form BEFORE you come for the physical.

If you have Asthma or Severe Food/Latex Allergies, additional forms will be needed/please see one of the school nurses.
